π§ PILOCYTIC ASTROCYTOMA vs HEMANGIOBLASTOMA
Two classic posterior-fossa lesions that can look remarkably similar on MRI: cyst + enhancing mural nodule.
π΅ Pilocytic Astrocytoma (PA)
β’ Usually children/adolescents
β’ Well-circumscribed cerebellar lesion
β’ Cyst + enhancing mural nodule
β’ Cyst wall may enhance
β’ Usually less vascular; flow voids uncommon
β’ High ADC, without marked diffusion restriction
π΄ Hemangioblastoma (HB)
β’ Typically young/middle-aged adults
β’ Cyst + intensely enhancing mural nodule
β’ Highly vascular β prominent flow voids
β’ Markedly elevated rCBV
β’ Cyst wall typically non-enhancing
β’ Consider VHL, especially with multiple lesions or young age
π Radiology pearl: A cerebellar cystic lesion with a brilliantly enhancing mural nodule + prominent flow voids/high rCBV should raise suspicion for hemangioblastoma.
π Concise differential diagnosis:
β’ Pilocytic astrocytoma
β’ Hemangioblastoma
β’ Metastasis
β’ Medulloblastoma
β’ High-grade glioma
β’ Ependymoma
β’ Abscess/infective lesion
π Based on concepts from Youmans & Winn Neurological Surgery and clinical localization principles from DeJongβs Neurologic Examination.
