ESMO 2026 | Rapid Oral Session: Key Updates in Upper GI Oncology
The Rapid Oral sessions at ESMO 2026 highlighted emerging treatment strategies across hepatocellular carcinoma (HCC) and biliary tract cancer (BTC), from cellular therapies to targeted treatments and immunotherapy combinations.
Key Research Highlights
**1. GPC3-Targeted CAR-T in Advanced HCC**
Early-phase findings with an investigational, GPC3-targeted armored CAR-T therapy showed encouraging antitumour activity in previously treated hepatocellular carcinoma. Cytokine release syndrome remains an important safety consideration.
**2. LEAP-012: Pembrolizumab, Lenvatinib and TACE**
Long-term results did not demonstrate an overall-survival benefit from adding pembrolizumab and lenvatinib to transarterial chemoembolization (TACE), despite previously reported improvements in progression-free survival.
**3. KRAS G12D-Targeted Treatment Strategies**
KRAS G12D inhibitors and combination approaches remain an important area of investigation. Study-specific efficacy results need to be interpreted in the context of the drug, tumour type and trial design.
**4. OPTIMUM: Maintenance Therapy in Biliary Tract Cancer**
The study explores olaparib alone versus olaparib combined with durvalumab in advanced biliary tract cancer with DNA damage repair gene alterations. The findings may help inform future research into biomarker-guided maintenance strategies.
**5. Envafolimab Plus GEMOX in Advanced Biliary Tract Cancer**
Clinical research has evaluated subcutaneous PD-L1 inhibition with gemcitabine and oxaliplatin. Published Phase III results reported improved overall survival compared with GEMOX alone, supporting further discussion of immunotherapy-based treatment strategies.
These developments highlight the diversity of approaches being investigated in gastrointestinal oncology. Equally important is the careful interpretation of clinical trial outcomes, safety profiles and biomarker-defined patient populations.
Scientific meetings such as ESMO offer an important platform for evaluating emerging evidence and discussing the future direction of cancer care.
