Summary Statement of Disability
I have multi‑system chronic medical conditions including coronary artery disease with reduced ejection fraction, COPD with persistent respiratory distress, recurrent DVT with chronic leg pain and swelling, musculoskeletal injuries causing mobility limitations, and severe depression/anxiety impairing cognitive and emotional functioning.
I experience shortness of breath, chest pain, leg pain, numbness, dizziness, fatigue, and repeated infections, all of which significantly limit my ability to stand, walk, lift, concentrate, and sustain full‑time employment.
My conditions are persistent, medically documented, and worsening, with objective findings including Heart Left ventricular EF
(Ejection Fraction) 45–55%, recurrent DVT, chronic cellulitis, and moderate‑severe mental‑health scores.
I have functional limitations consistent with disability, including inability to perform prolonged standing, walking, lifting, or reliable attendance due to respiratory distress, vascular pain, and mental‑health instability.
