AV fistula. Very rightly, this is the literal lifeline for dialysis patients. As a nephrologist, I cannot stress enough on the importance of a good functioning AVF. It could well be the difference between life and death for a dialysis patient. Best practice guidelines unequivocally recommend preparing a potential CKD (Chronic Kidney Disease) patient for impending dialysis. This, among other things, involves establishing a stable, reliable vascular access, when patient is in advanced stages of CKD. But ever so often I come across patients who simply refuse to accept their diagnosis and acknowledge need for future dialysis. Just today, one of my patients, well along in years, with stage 5 CKD, flatly refused to even begin a discussion on fistula planning. I hope it's not a missed opportunity, and next time around I will get a breakthrough. Fingers crossed 🤞...
