Chemical accidents. Oil spills. Nuclear accidents. Earthquakes, floods and wildfires. Contaminated towns. Safety systems that failed.
Each case is rebuilt from the facts: what happened, what was known before it happened, which safeguards failed, how authorities responded, and what changed afterward.
No sensationalism, no conspiracy theories, no simple villains. Real events, real sources, and one question: how does a system that appears under control become a catastrophe?
