✨🎓Question: A 36-year-old in her third pregnancy has a BMI of 37 at her first visit and no other risk factors. What is correct under NG133?
A. Advise aspirin, because a BMI of 35 or more is a high-risk factor
B. Advise aspirin, because any one moderate factor is enough
C. Aspirin is advised only with more than one moderate factor, so not on these criteria alone (correct)
D. Advise aspirin 150 mg from 20 weeks
Explanation:😎 A BMI of 35 or more at the first visit is one moderate risk factor. NG133 advises aspirin for more than one moderate factor, or for any one high-risk factor.
Live Tomorrow at [9 pm Cairo time]: Round 1, hypertension in pregnancy, 17 SBAs.

💊 Aspirin in pregnancy: who gets it, how much, and from when?
A favourite from NICE NG133. Look at the picture, then test yourself.
HIGH RISK (one factor is enough)
Chronic hypertension · chronic kidney disease · type 1 or 2 diabetes · autoimmune disease (SLE, antiphospholipid syndrome) · hypertensive disease in a previous pregnancy
MODERATE RISK (two or more)
First pregnancy · age 40 or older · pregnancy interval over 10 years · BMI 35 or more at first visit · family history of pre-eclampsia · twins or triplets
THE REGIMEN
75 mg to 150 mg once daily, from 12 weeks until birth.
The trap: one high-risk factor is enough, but moderate factors need two or more.
❓ Can you answer this? A 41-year-old in her first pregnancy, BMI 36 at booking, no other risk factors. Aspirin or not? Answer in the comments.

🚨 The Golden Hour starts NOW.
🚨Your Para 2 patient is 3 weeks post-miscarriage and spiking a 38.5°C fever.
In the post-pregnancy period, robust physiological reserve can mask early shock—meaning if you wait for classic signs of cardiovascular collapse, you are already too late.
The Obstetric Sepsis Six is your non-negotiable lifeline. But in the emergency room, sequence matters just as much as speed.
Examine the Sepsis Six wheel below. 👇
🧠😎Clinical Question: Which specific step MUST be completed before you push that first dose of broad-spectrum IV antibiotics?Drop your answer and your clinical reasoning in the comments! 💬

🚨 38.5°C Fever, 3 Weeks Post-Miscarriage. What’s your next move? 🚨
You are covering the acute gynecology unit. A Para 2 patient presents with a temperature of 38.5°C exactly three weeks after a spontaneous miscarriage. She reports feeling generally unwell with mild pelvic discomfort.
With the physiological adaptations of pregnancy still resolving, her baseline parameters can mask early systemic inflammatory response syndrome (SIRS). According to the latest MBRRACE-UK reports and RCOG/NICE guidelines, maternal sepsis remains a leading cause of mortality—and the clock is ticking.
Before we dive into the full case breakdown in today’s video, what is your most critical immediate diagnostic step?
👇 Vote below and defend your answer in the comments! 👇
A) Immediate Transvaginal Ultrasound for RPOC
B) Point-of-care serum lactate & blood cultures
C) Start empiric Piperacillin-Tazobactam
D) Pelvic CT to rule out Septic Pelvic Thrombophlebitis
