1) Find out how much is vested. Unvested funds are not yours.
2) Do not cash it out unless you are at retirement or in a real emergency.
3) Compare your options. Leave it, move it to a new plan, or roll it to an IRA.
In most cases the IRA rollover wins. Better control, more investments options, lower fees, and the ability to do Roth conversions on your own terms.
