ABOUT VETERINARY MEDICINE EDUCATION
Friends, I'm going to ask you some questions about the coursework and your undergraduate veterinary medicine education in general. I want you to answer these questions honestly, thoughtfully, logically, conscientiously, and based on your knowledge (or research if you lack knowledge), writing them in short bullet points. Express yourself.
QUESTIONS
1) Do you think a veterinarian who doesn't plan to practice veterinary medicine after graduation still carries the responsibility of being a doctor? When people and institutions question them based on their diplomas, do they feel responsible for the lives, health, and well-being of both animals and humans? If not, what happens? Is passing courses more important than acquiring knowledge and becoming a doctor? In this respect, what is gained or lost by passing courses by memorizing or cheating?
2) You are working in the field. You are a veterinarian. You don't have much money, so you travel from village to village providing treatment. You are preparing to perform a necropsy. The animal owner – a villager, uneducated, cunning – tells you: "After the necropsy, I'll take the meat and bones and sell them, or we'll eat them as a family." Would you allow this? Or, "Okay, I won't eat or sell them, but at least I'll give the meat and bones to my dog." Would you allow this? Why yes? Why no? What diseases are there, and what are the risks of transmission?
3) What are the risks of taking the easy way out and accepting/believing the medical history given by the animal owner as is? Could they deceive you? Can you be sure that the animal owner won't go to villages, see the names of the medicines you've prepared, find these medicines on their own, and inject them into sick animals without knowing the correct dosage?
4) In the future, when you become a doctor and work in any field, even if it's not related to your specialty, what advantages or disadvantages would your developed or undeveloped skills in systematic thinking, critical thinking, respecting your teachers, elders, and experts, and taking or ignoring their advice have created in your life?
5) Do you think learning is something that happens instantly or is a process? Can focusing your attention, concentration, and willpower on a task, both in and out of school, and constantly thinking about it (as in language learning), or occasionally reviewing lessons outside of school, or referring back to your book out of curiosity, help develop creativity and focus, benefiting your studies, professional life, and character development?

Mechanism of rigor mortis formation:
In some respects, the formation of rigor mortis is similar to intravital muscle contraction.
1) The contraction event begins with the influx of calcium from the sarcoplasmic reticulum into the sarcoplasm.
2) ATP is used as the energy source.
3) Muscle stimulation occurs via nerves. The permeability of the perisynaptic membrane covering the nerve ending increases. Acetylcholine is released into this synapse gap. By stimulating the postsynaptic membrane, a flow of sodium ions from the cell membrane to the muscle fiber begins. The stimulation provided by this ion spreads to the region of the T tubules, which invaginate into the muscle from the sarcolemmine. T tubules extend to the Z band. In this way, this stimulation extends to the cisterns formed by the reticulum. Calcium ions and ATP are released from the cisterns. ADP and energy are released from ATP. Thanks to this energy, actin and myosin filaments approach each other and interlock like fingers, forming actinomyosin, and muscle contraction occurs.
For muscle relaxation, calcium ions must return to the smooth endoplasmic reticulum.

Why should carcasses of large animals suspected of having anthrax never be opened?

The main reason is to prevent massive contamination (infection) of the environment and people with this dangerous disease.
Anthrax (Bacillus anthracis) is a zoonotic disease that can affect all mammals, including humans, and leads to different clinical presentations depending on the route of transmission. In humans, the course of infection is seen in three main forms depending on the route of entry of the agent into the body:
*Cutanical Anthrax: This is the most common form in humans and accounts for more than 95% of cases. It is usually transmitted through areas where the integrity of the skin is compromised as a result of contact with infected animals, carcasses, or animal products. Symptoms: It begins as an itchy papule (raised bump) 3 to 5 days after infection. This structure quickly transforms into a bloody vesicle, which then ruptures, forming a characteristic lesion called a carbuncle, characterized by a black, hard crust (eschar) in the center and redness around it. Widespread edema around the lesion is typical in the early stages.
* Respiratory Anthrax (Pulmonary Form): Also known as "woolsorter's disease" in humans. It occurs through inhalation of airborne anthrax spores. This form is highly risky and a critical agent in terms of bioterrorism.
* Digestive Anthrax (Alimentery Form): Occurs through the consumption of contaminated meat or animal products (ingestation). In this form, bacteria penetrate the intestinal epithelium and initiate the infection. Symptoms: Manifests with symptoms such as severe abdominal pain, vomiting blood (hematemesis), and high fever. All forms of anthrax can be life-threatening due to the potent toxins produced by the bacteria, leading to vascular damage, edema, and tissue necrosis.
The anthrax-causing agent, Bacillus anthracis, rapidly forms highly resistant spores when a carcass is opened and exposed to air. However, when the carcass remains closed, the lack of oxygen in the environment limits this sporulation.
These spores are exceptionally resistant to extreme environmental conditions (heat, drought, etc.) and retain their infectious properties for years.
Opening the carcass causes the agent to spread over a very wide area to other animals and humans through air, water, or soil (zoonotic risk).
In animals that die from anthrax, the blood loses its clotting ability, and tar-like, dark-colored blood seeps from natural orifices (mouth, nose, anus); this blood contains a large amount of bacteria, and any contact with it is risky.
Instead of dissecting the cadaver, a small blood sample should be taken from the ear vein or, in carnivores, from the swollen head/neck area for cytological examination or microbiological culture to make a diagnosis. The presence of large, encapsulated, blunt-ended bacterial rods in blood smears is sufficient for diagnosis.

*Deri Şarbonu (Kutanöz Form): İnsanlarda en sık rastlanan formdur ve vakaların %95'inden fazlasını oluşturur. Genellikle enfekte hayvanlarla, karkaslarla veya hayvansal ürünlerle temas sonucu deri bütünlüğünün bozulduğu bölgelerden bulaşır.Belirtileri: Enfeksiyondan 3 ila 5 gün sonra kaşıntılı bir papül (kabartı) şeklinde başlar. Bu yapı kısa sürede kanlı bir veziküle (kesecik) dönüşür, ardından bu vezikül patlayarak merkezinde siyah, sert bir kabuk (eşar) ve çevresinde kızarıklık bulunan, karbonkül adı verilen karakteristik lezyonu oluşturur. Erken aşamalarda lezyon çevresinde yaygın ödem görülmesi tipiktir.
* Solunum Şarbonu (Akciğer/Pulmoner Form): İnsanlarda "yüncü hastalığı" (woolsorter’s disease) olarak da adlandırılır. Havada asılı kalan şarbon sporlarının solunmasıyla (inhalasyon) meydana gelir. Bu form oldukça risklidir ve biyoterörizm açısından da kritik bir ajandır.
*Sindirim Şarbonu (Alimenter Form): Kontamine olmuş etlerin veya hayvansal gıdaların tüketilmesiyle (ingestiyon) oluşur. Bu formda bakteriler bağırsak epitelini geçerek enfeksiyonu başlatır. Belirtileri: Şiddetli karın ağrısı, kan kusma (hematemez) ve yüksek ateş gibi semptomlarla kendini belli eder.
Şarbonun her formu, bakterinin ürettiği güçlü toksinler nedeniyle damar hasarı, ödem ve doku nekrozuna yol açarak hayati tehlike oluşturabilir.
-Şarbon etkeni olan Bacillus anthracis, kadavra açılıp havayla temas ettiğinde hızla son derece dayanıklı sporlar oluşturur. Kadavra kapalı kaldığında ise ortamın oksijensiz olması bu sporlanmayı sınırlar.
-Bu sporlar çevresel ekstrem koşullara (sıcaklık, kuraklık vb.) karşı olağanüstü dirençlidir ve yıllarca bulaşıcı özelliklerini korurlar.
-Kadavranın açılması, etkenin hava, su veya toprak yoluyla diğer hayvanlara ve insanlara (zoonoz risk) çok geniş bir alanda yayılmasına neden olur.
-Şarbondan ölen hayvanlarda kanın pıhtılaşma özelliği kaybolur ve doğal deliklerden (ağız, burun, anüs) katran benzeri koyu renkli kan sızar; bu kan bol miktarda bakteri içerir ve her türlü temas risklidir.
-Kadavrayı açmak yerine, tanı koymak amacıyla kulak venasından (toplardamarından) veya karnivorlarda şişkin olan baş/boyun bölgelerinden küçük bir miktar kan örneği alınarak sitolojik muayene veya mikrobiyolojik ekim yapılmalıdır. Kan frotilerinde kapsüllü, küt uçlu büyük bakteri çomaklarının görülmesi tanı için yeterlidir.

1. Anthrax: This is the most emphasized and highest-risk disease in the literature; necropsy should absolutely not be performed on carcasses suspected of having anthrax. The causative agent, B. anthracis spores, are extremely resistant to environmental conditions and are a source of infection for humans. In ruminants, the spleen is enlarged and takes on a tar-like appearance.
2. Rabies: This is one of the primary reasons for requesting a necropsy. In suspected cases, additional protective measures beyond routine postmortem examination should be taken, such as double gloves, masks, eye protection, and adequate ventilation.
3. Tuberculosis (Mycobacterium bovis): This is a chronic and contagious risk worldwide for humans and domestic animals. Unpasteurized milk and infected cattle are particularly important sources of tuberculosis for humans. In cattle, it is characterized by granulomas (tubercles) containing nodules and caseous necrosis in the lungs.
4. Glanders: A contagious disease affecting horses, notifiable to the OIE, and often fatal in humans if left untreated. It can be transmitted to carnivores and humans through the consumption of infected horse meat.
5. Brucellosis: A disease affecting horses, ruminants, and pigs, causing reproductive system disorders, and transmissible to humans.
6. Echinococcus (Hydatid Cyst): A zoonotic parasite characterized by clear fluid-filled cysts in the liver, which should be considered during necropsy.
7. Salmonellosis: Certain species in poultry can cause infection in humans, making it zoonotic. It can also cause septicemia in cattle, sheep, goats, and pigs.
8. Tularemia: When a zoonotic agent is suspected, the necropsy should be directly referred to a diagnostic laboratory.
