Cozen's Test for Lateral Epicondylitis (Tennis Elbow) #shorts Not every sore elbow is #tenniselbow but when a patient points to the bony bump on the outside of the elbow with pain, Cozen's test is can help. Lateral epicondylitis is an overuse tendinopathy at the origin of the wrist extensor tendons where they attach to the lateral epicondyle. Despite the name, most people who develop it never pick up a racket. Repetitive gripping, twisting, and wrist extension are the real culprits: plumbers, painters, carpenters, cooks, and anyone who spends long hours on a computer mouse. Often there is no single event to blame. . Patients describe a sharp or burning pain on the outside of the elbow that can travel down the forearm into the wrist. Grip weakens. Turning a doorknob, shaking hands, or holding a coffee cup becomes surprisingly difficult. . Cozen's test is simple and reproducible. Sit with your elbow extended, forearm pronated, wrist radially deviated, and hand in a fist. We stabilize your elbow with one hand while palpating the lateral epicondyle, place our other hand on the back of your fist, and ask you to extend your wrist against resistance. Pain reproduced at your lateral epicondyle is a positive test. Two refinements improve accuracy: Keeping fingers flexed reduces contribution from the extensor digitorum origin, and applying resistance on the dorsoradial side of your fist limits involvement of extensor carpi ulnaris, which can create a false positive. Cozen's is a sensitive test - a negative result helps rule tennis elbow out. It's not a standalone diagnosis and often paired with Mill's and Maudsley's tests, a grip strength comparison, +/- imaging when the picture is unclear.
