The Intramedullary Nail: An 80-Year-Old Idea Surgeons Still Use Daily #shorts . In 1939, a German surgeon named Gerhard KΓΌntscher did something his colleagues considered reckless: he drove a metal rod down the center of a fractured femur and told the patient to get up and walk. At the time, a broken thigh bone meant weeks in traction and months in plaster. KΓΌntscher's insight was that the medullary canal itself could carry the load. His cloverleaf-shaped nail was flexible enough to slide down the canal yet stiff enough to hold alignment, giving stability from the inside out with far less soft tissue disruption than open plating. The technique spread in an unusual way. Allied prisoners of war treated in German hospitals came home in 1945 with nails in their femurs, and Western surgeons saw the results before they read the papers. KΓΌntscher later added reaming to improve fit and fixation, a concept we still rely on. Today's titanium nails are refined, but the principle is his: fix the fracture from within and let the patient move. As an orthopedic surgeon, I still find that a remarkable act of conviction. .
